髋关节疾病是小儿骨科最常见、最复杂的问题之一,通常需要手术治疗。儿童髋关节手术的术中和术后麻醉较为保守,主要限于硬膜外麻醉,尽管麻醉质量较高,但其缺点和局限性众所周知。髋关节手术中的外周区域阻滞在成人中应用广泛。超声导航的出现使周围阻滞能够更安全、更有效地进行, 这影响了其在儿童中的应用,也促进了筋膜间阻滞新方法的应用。作者分析了有关儿童髋关节手术中使用的外周区域阻断现代方法的有效性和安全性的研究。文献检索在eLibrary、PubMed和Scopus数据库中进行。利用关键词找到了750篇文献资料,综述中收录与研究目的相符的文献65篇。研究最多的儿童区域技术是腰丛阻滞和髂筋膜阻滞,其镇痛效果并不亚于腰部硬膜外麻醉和尾部麻醉。 髋关节周围神经阻滞是一种新的、有前途的髋关节区域麻醉方法,在成人患者中已经得到了很好的研究。然而,迄今为止,文献中描述的用于儿科髋关节手术麻醉的外周区域麻醉技术均未证明优于其他技术。我们需要进一步的临床研究来调查外周区域麻醉技术,并确定其在小儿髋关节手术中的有效性和安全性。
