克罗恩病是一种慢性复发的肠道炎症性疾病,伴随高风险的系统性和局部并发症。在儿童中,每年新发病例持续增加。尽管处于生物治疗和新疗法的时代,手术治疗的作用仍然十分重要。
研究目的。分析儿童Crohn病回盲区局部损害的手术治疗现代趋势和方法。文献搜索在以下科学数据库进行:eLibrary、Google Scholar、Medline 和 Embase,关键词包括:“Crohn病”、“Crohn病手术治疗”、“早期回盲区切除”、“Kono-S吻合术”。搜索时间范围为2017年至2024年。近年来的一些研究显示,针对Crohn病回盲区的早期手术治疗趋势逐渐增强。这种方法在多项成人研究中被视为药物治疗的替代选择。早期手术治疗的优势体现在减少术后并发症、改善患者生活质量以及降低治疗成本上。不同作者对“早期手术”的定义和时间框架有所不同。 已知儿童期的Crohn病具有更为侵袭性的特点,因此,较早进行手术治疗可能不仅能提供更好的疾病控制机会,还能改善长期治疗效果。继续研究手术技术的作用,包括是否需要将伴有脂肪变性的肠系膜纳入切除范围,以及吻合方式对Crohn病复发的影响。对Kono-S吻合术的安全性分析及其对局部复发的影响显示出相较于传统方法的多项优势,但在儿童外科和结肠直肠外科的应用经验仍较为有限。需要在儿童中开展类似研究,以确定治疗局部回盲区损害的Crohn病的最佳策略。这包括评估手术治疗的作用及肠道吻合技术在实现长期和深度缓解中的影响,尤其是在炎性表型的Crohn病中,同时防止疾病复发和再次切除。
