Антитромбоцитарная терапия: вторичная профилактика ишемического инсульта

Аннотация
Перенесенный эпизод острой церебральной ишемии, даже обратимой, является важным прогностическим фактором возникновения ИИ. Как свидетельствуют результаты популяционных исследований, наиболее опасными в плане развития повторного ИИ являются первые дни и недели после перенесенного эпизода острой церебральной ишемии [17]. Вместе с тем риск сохраняется и в последующем, хотя и в несколько меньшей степени. Имеются данные о том, что эпизод острой церебральной ишемии ассоциирован с последующим риском развития как ИИ, так и острой коронарной патологии, хотя более высока вероятность развития ИИ, чем возникновения инфаркта миокарда (ИМ) [17, 32]. Вероятность развития повторного ИИ определяется рядом факторов, в первую очередь его патогенетическим подтипом. Имеются данные о том, что при стенозирующем атеросклеротическом поражении крупных экстраи интракраниальных артерий риск ИИ в 8 раз выше, чем при микроангиопатии [36]. Очевидно, что в группе более высокого риска развития ИИ находятся пациенты с критическим стенозом одной (тем более обеих) внутренних сонных артерий, мультифокальным атеросклеротическим поражением крупных артерий головы [3]. Вместе с тем риск повторного ИИ связан с наличием ряда факторов риска, как потенциально модифицируемых, так и немодифицируемых, тщательный учет которых способен с высокой точностью предсказать вероятность развития повторного ИИ [47]. В настоящее время в качестве важного прогностического фактора, указывающего на возможность развития ИИ, рассматривают наличие выявленных очагов «немых» («молчаливых») инсультов, которые четко визуализируются по результатам магнитно-резонансной томографии (МРТ), но ранее никак не проявлялись клинически.
