Аннотация
Россия занимает одно из первых мест в мире по сердечно-сосудистой смертности (среди мужчин она достигает 1500 исходов на 100 тыс. человек). Ведущими причинами столь неблагоприятной статистики являются ишемическая болезнь сердца (ИБС) и инсульт. Антитромбоцитарная терапия сегодня является необходимым звеном полноценной схемы лечения пациентов с высоким риском сердечно-сосудистых осложнений, включая больных с установленной ИБС. На протяжении десятилетий антиагрегантом первого ряда служит ацетилсалициловая кислота (АСК). Механизм действия АСК детально изучен и заключается в необратимом ингибировании активности циклооксигеназы тромбоцитов с последующим уменьшением синтеза тромбоксана А2 и простациклина. АСК блокирует реакцию освобождения тромбоцитов, индуцированную АТФ и норадреналином. Тромбоциты не имеют ядра и, соответственно, не могут синтезировать белок. Поэтому ингибирование циклооксигеназы не обратимо и сохраняется в течение 7–10 дней (продолжительность жизни тромбоцита). Последние исследования доказали влияние АСК на эндотелиальную дисфункцию путем образования индуцибельной синтазы оксида азота и уменьшения активности супероксиданиона, что в значительной степени ограничивает оксидативный стресс.
