Анемия при диабетической нефропатии: прогностическое значение, диагностика и лечение

Аннотация
Анемия как следствие сниженного синтеза ЭПО интерстицием почек является достаточно ранним осложнением СД, развиваясь у каждого пятого больного с МАУ и нормальным уровнем СКФ (более 90 мл/мин). В отличие от нефропатий недиабетического генеза анемия при СД характеризуется более тяжелым течением и неадекватно низким синтезом ЭПО в ответ на сниженные значения уровня гемоглобина. Анемия при СД способствует более быстрому прогрессированию как микро-, так и макрососудистых осложнений, повышая риск гибели больных от сердечно-сосудистых катастроф. Имеются данные, что своевременная коррекция почечной анемии с помощью генно-инженерных препаратов ЭПО позволяет избежать тяжелых сосудистых осложнений и повысить качество жизни больных СД. Наиболее клинически апробированным препаратом для лечения почечной анемии при подкожном введении является эпоэтин бета. Для оценки роли коррекции анемии в профилактике сердечно-сосудистых осложнений и ее влиянии на прогрессирование нефропатии в настоящее время проводится несколько международных рандомизированных исследований (ACORD, CREATE, TREAT), которые также позволят решить вопрос о том, когда необходимо начинать лечение анемии и каков целевой уровень гемоглобина.
