Аннотация

Тромбоз мозговых вен, или церебральный венозный тромбоз (ЦВТ) – это редкое цереброваскулярное заболевание и неспецифический тип инсульта, на долю которого приходится 0,5–1% всех случаев инсульта. ЦВТ чаще встречается у женщин, чем у мужчин. Это связано с наличием таких факторов риска, как беременность, послеродовой период и использование оральных контрацептивов. Клинические симптомы возникают в результате повышения внутричерепного давления, вызванного нарушением венозного оттока и повреждением головного мозга. ЦВТ связан с широким спектром клинических симптомов от изолированных головных болей (наиболее частый симптом при поступлении, регистрируемый в 90% случаев) до фокальных дефицитов, судорог и изменений психического статуса. Диагноз ЦВТ считается одним из самых ошибочно диагностируемых в клинической неврологии, поскольку его гетерогенное проявление может быть как у молодых, так и у пожилых людей. Тщательный сбор анамнеза с прицельным выявлением факторов риска, проведение полноценной диагностики и лечения способствуют достижению благоприятного клинического исхода у пациентов. Акцент в статье сделан на степени реканализации ЦВТ, оценке венозных коллатералей по степени Qureshi и клинических исходах в зависимости от факторов риска. Эти показатели мало изучены в настоящее время.
