In its advanced form, median survival rarely exceeds 12 months, even with chemotherapy. However, in recent years, the advent of immune checkpoint inhibitors (PD-L1 inhibitors), such as atezolizumab, has improved survival rates while maintaining acceptable toxicity.</p> <p><bold>Description</bold><bold> </bold><bold>of</bold><bold> </bold><bold>the</bold><bold> </bold><bold>clinical</bold><bold> </bold><bold>case</bold><bold>: </bold>This case report presents the experience of treating a patient with disseminated small cell lung cancer. She achieved a significant positive response to combination chemoimmunotherapy (etoposide + carboplatin + atezolizumab) followed by maintenance therapy with an anti-PDL1 inhibitor. A brief overview of clinical trials on the efficacy of PD-L1 blockers in the treatment of SCLC is also provided.</p> <p><bold>Conclusion</bold><bold>: </bold>The combination of immunotherapy and chemotherapy has become the standard of care for small cell lung carcinoma. The high efficacy of this combination was demonstrated in our clinical case.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Обоснование:</bold> Мелкоклеточный рак лёгкого (МРЛ) – злокачественное новообразование с агрессивным течением и крайне неблагоприятным прогнозом. При распространённой форме – медиана выживаемости редко превышает 12 месяцев даже при проведении химиотерапевтического лечения. Тем не менее в последние годы появление ингибиторов иммунных контрольных точек (ингибиторов PD-L1), таких как атезолизумаб, позволило достичь улучшения показателей выживаемости при сохранении приемлемого уровня токсичности.</p> <p><bold>Описание клинического случая:</bold> В данном клиническом случае представлен опыт лечения пациентки с диссеминированным мелкоклеточным раком лёгкого, у которой достигнут выраженный положительный эффект на фоне комбинированной химиоиммунотерапии (этопозид+карбоплатин+атезолизумаб) с последующим поддерживающим лечением анти-PDL1 ингибитором. Также приводится краткий обзор клинических исследований по эффективности PD-L1-блокаторов в лечении МРЛ.</p> <p><bold>Заключение: </bold>Комбинация иммунотерапии с химиотерапией стала стандартом лечения мелкоклеточной карциномы лёгких.
