Боль после кесарева сечения: есть ли у нас надёжные предикторы? Обзор предметного поля

Аннотация
Обоснование. С каждым годом растет число публикаций, посвященных изучению различных инструментов прогнозирования интенсивности боли после кесарева сечения (КС), что обусловило необходимость обобщения и систематизации этих данных.
Цель. Нашей целью было определить факторы, способствующие развитию высокоинтенсивной боли после операции КС.
Методы. Обзор предметного поля, основанный на рекомендациях PRISMA for Scoping Reviews (PRISMA-ScR), проведён с использованием источников из баз данных PubMed, Cochrane Database of Systematic Reviews и поисковой системы Google Scholar. Поиск выполняли по следующим ключевым словам на русском и английском языке: «предикторы» ИЛИ «предикция» ИЛИ «прогнозирование» И «кесарево сечение» И «боль» («predictors» OR «prediction» OR «forecasting» AND «cesarean section» AND «pain»). Дата последнего поискового запроса — 30.11.2022. Критериями включения в обзор послужили следующие (по методу PICOD): (P) популяция — родильницы; (I) вмешательство — операция КС; (C) сравнение — хирургический доступ, метод анестезии, психологический статус, порог боли, генетические характеристики; (O) исходы — показатели интенсивности боли, потребность в анальгетиках; (D) дизайн исследования — про-/ретроспективные когортные исследования. Критерии исключения: отсутствие достаточных данных или интересующих результатов; дублирующая публикация; хроническая боль; публикации, посвящённые обезболиванию родов или боли после других оперативных вмешательств; отсутствие полнотекстовой версии; обзоры и метаанализы. Оценку качества отобранных нерандомизированных когортных исследований выполняли с использованием шкалы Ньюкасла–Оттавы (Newcastle–Ottawa Scale, NOS).
Результаты. Было отобрано 30 когортных исследований, в которых приняли участие 11 063 пациентки. Большинству исследований было присвоено более 6 баллов по NOS, что соответствовало хорошему качеству. В качестве предикторов интенсивности послеоперационной боли было выделено 2 группы факторов: факторы, связанные с характеристикой пациента (физический статус, психологический статус, индивидуальный болевой порог и терпимость боли, генетические особенности), и факторы, ассоциированные с особенностями операции и анестезии.
Заключение. Обзор предметного поля позволил установить надёжные факторы предикции высокоинтенсивной боли после КС, которые должны быть учтены при планировании анестезиологического обеспечения пациенток.
