Трансуретральная резекция единым блоком (en bloc) при немышечно-инвазивном раке мочевого пузыря

Рак мочевого пузыря (РМП) – тяжелое, в ряде случаев инвалидизирующее заболевание, для которого не разработано системы активного выявления, требующее тщательной дифференциальной диагностики, имеющее большую склонность к рецидивированию и прогрессированию. Выбор оптимального метода лечения немышечно-инвазивного рака мочевого пузыря (НМИРМП) может снижать риск рецидива опухоли и улучшать результаты онкологической выживаемости. Создание протокола Vesical Imaging Reporting and Data System (VI-RADS), который обладает высокой чувствительностью и специфичностью для оценки степени инвазии опухоли в мышечную стенку, меняет парадигму к первичному оперативному лечению. Использование новых шкал визуализации при магнитно-резонансной томографии (МРТ) VI-RADS и интраоперационных протоколов (шкала глубины эндоскопической перфорации DEpth of Endoscopic Perforation [DEEP]) определяет выбор тактики лечения. Частота наличия мышечного слоя при en bloc-резекции – 96–100%. Доказано отсутствие циркулирующих опухолевых клеток в периферическом венозном кровотоке при выполнении en bloc-резекции по сравнению с классической трансуретральной резекцией (ТУР). Профиль безопасности и патоморфологический препарат лучше при использовании лазерной энергии. Повторная трансуретральная резекция после выполненной en bloc-резекции с наличием детрузора в патоморфологическом препарате и отрицательным хирургическим краем остается на усмотрение специалиста. Патоморфологический анализ, полученный при en bloc-резекции мочевого пузыря, позволяет более точно устанавливать стадию заболевания и определять выбор лечебной тактики при раке мочевого пузыря. Трансуретральная резекция единым блоком может быть рекомендована как стандартная процедура для диагностики и лечения НМИРМП.
