Роль вируса папилломы человека в развитии потенциально злокачественных заболеваний и плоскоклеточных карцином слизистой оболочки полости рта

Аннотация
Распространенность вируса папилломы человека (ВПЧ) при потенциально злокачественных заболеваниях (ПЗЗ) полости рта составляет 22,5%. Различные типы ВПЧ характеризуются генотипическими вариациями в последовательностях оснований ДНК E6 и E7. Именно эти генотипические различия позволяют разделить онкогенный фенотип вируса на типы высокого и низкого онкологического риска. Выделяют местные и общие факторы риска развития инфекции ВПЧ. К местным факторам риска относятся плохая гигиена полости рта, использование полных зубных протезов в сочетании с пожилым возрастом. Подтверждена значительная корреляция между гигиеной полости рта и вирусной нагрузкой. В данном обзоре приведена возможная роль вируса папилломы человека (ВПЧ) в развитии потенциально злокачественных заболеваний и плоскоклеточных карцином полости рта. Особое внимание уделено эпидемиологии, факторам риска и механизмам развития папилломавирусной инфекции. Рассмотрены клинико-морфологические различия между ВПЧ-позитивной и ВПЧ-негативной формами рака полости рта. Освещены вопросы профилактики инфекции ВПЧ и разработки патогенетической терапии данного заболевания. Поиск литературы осуществлялся в поисковых системах Medline, elibrary.ru, Scopus, PubMed, The Cochrane Library, РИНЦ.
