Радиационно индуцированные оральные мукозиты — патогенез, факторы риска, клинические проявления, профилактика, лечение

Аннотация
Изложены современные представления о патогенезе, факторах риска, этиологии, клинических проявлениях, средствах профилактики и лечения орофарингеального мукозита — одного из основных осложнений лучевой терапии рака, локализованного в области головы и шеи. Показано, что в механизмах развития заболевания ведущую роль играет повреждение быстроделящихся клеток базального слоя слизистой оболочки ротоглотки, обусловленное как прямым (возбуждение и ионизация) так и косвенным (накопление активных соединений кислорода и азота, активация транскрипционных факторов, гиперфункция провоспалительных цитокинов и др.), действием ионизирующих излучений на важнейшие биологические молекулы и надмолекулярные структуры (полинуклеотиды, нуклеопротеиды, фосфолипиды и др.). Основными факторами риска мукозита являются локализация и размер опухоли, величина дозы облучения оральной области, индивидуальные особенности организма (молодой возраст, вредные привычки, металлические конструкции зубных протезов, наличие в анамнезе болезней пародонта и др.). Описаны степени тяжести заболевания: от легкой (эритема и умеренно выраженная боль, не требующая анальгезии) до тяжелой — изъязвления, геморрагии, некроз и другие симптомы, для установления которых необходимо назначение эффективных средств патогенетической и симптоматической терапии. Перечислены основные средства и методы профилактики радиационно индуцированного мукозита: нестероидное противовоспалительное средство бензидамин; средство защиты от оксидативного стресса глютамин; препараты, обладающие комплексным стимулирующим действием на процессы заживления ран. Медикаментозная терапия мукозита предусматривает применение бензидамина; трициклического антидепрессанта доксепина; геля, состоящего из поливинилпирролидона (Belclaiz), муколитика ацетилцистеина. При неэффективности ненаркотических анальгетиков используют низкие дозы опиатов (фентанил, морфин, метадон) в оральной, трансдермальной или парентеральной лекарственной формах, низкоинтенсивную лазерную терапию. Важнейшим компонентом этиопатогенетической терапии заболевания являются противовирусные препараты (ацикловир, валацикловир) и фунгициды (клотримазол, флуконазол, интраконазол). Затрудненное энтеральное питание является показанием для использования назогастрального или назоеюнального зондов, а в более серьезных случаях — применения хирургических методов — гастро- или еюностомии.
