Регуляторные Т-клетки CD4+CD25+FOXР3+CD127LOW у больных вульгарным псориазом

Аннотация
Цель. Патогенетическое обоснование роли регуляторных Т-клеток (Трег) у больных вульгарным псориазом (ВП) и обоснование их применения для диагностики, лечения и оценки эффективности терапии данного заболевания
Материалы и методы. Обследовали 60 больных ВП (35 женщин и 25 мужчин) в возрасте от 18 до 55 лет, из них 28 находились в прогрессирующей стадии, 19 – в стационарной, 13 – в регрессирующей. Псориазом менее 20 лет страдали 42, 20 лет и более – 28 больных, их них у 10 течение ВП было легкой степени тяжести, у 22 – средней, у 28 – тяжелой. До начала и по завершении курса терапии узкополосным ультрафиолетовым излучением В-спектра 311 нм производилось определение Трег в периферической крови у 12 больных, находившихся в прогрессирующей стадии ВП. В контрольную группу вошли 42 здоровых донора.
Результаты. Установлено, что количество CD4+CD25+Foxр3+CD127LOW Трег в крови больных ВП меньше, чем у здоровых доноров (2,84 ± 1,00 и 4,02 ± 0,73% соответственно). Также было продемонстрировано увеличение содержания Трег по мере перехода прогрессирующей стадии ВП (2,59 ± 0,68%) в стационарную (2,82 ± 1,55%) и затем в регрессирующую стадию (3,68 ± 1,62%). Выявлена обратная корреляция (r = -0,39) между показателями Трег в периферической крови и тяжестью течения ВП. Вместе с тем было установлено, что показатели Трег обратно пропорциональны (r = -0,46) длительности течения заболевания. Дополнительно на примере 12 пациентов была продемонстрирована способность курса фототерапии узкополосным ультрафиолетовым излучением (УФБ-311нм) увеличить содержание Трег в периферической крови больных ВП с 2,11 ± 0,61% до начала терапии до 3,43 ± 1,02% после неё.
