Роль полиморфизма гена эпидермального фактора роста в формировании тяжёлых проявлений кожной токсичности, вызванной применением EGFR-ингибиторов

Ингибиторы рецептора эпидермального фактора роста (epidermal growth factor receptor, EGFR) широко используются для лечения различных видов рака, таких как немелкоклеточный рак лёгких, рак головы и шеи, молочной железы, поджелудочной железы, колоректальный рак. Во время лечения могут возникать такие класс-опосредованные эффекты, как кожная токсичность, интерстициальное заболевание лёгких, гепатотоксичность, глазная токсичность, гипомагниемия, стоматит и диарея. Роль эпидермального фактора роста (epidermal growth factor, EGF) и его рецепторов в жизнедеятельности кератиноцитов является ключевой. Дерматологические нежелательные эффекты возникают у 60–90% пациентов, и зачастую сохраняются на протяжении всего периода терапии ингибиторами EGFR.
В настоящий момент исследования биомаркеров прогнозирования тяжести кожных осложнений терапии ингибиторами EGFR крайне ограничены. Таким образом, актуальным является изучение генетических маркеров, достоверно ассоциированных с развитием тяжёлой кожной токсичности на фоне терапии ингибиторами EGFR, поскольку сигнальный путь EGFR важен для поддержания нормальной физиологической функции кожи. Развитие тяжёлых кожных реакций приводит к снижению дозы или прекращению приёма ингибиторов EGFR при лечении рака. Недавно был достигнут прогресс в исследованиях кожной токсичности ингибиторов EGFR.
В статье мы кратко описываем механизм кожной токсичности, вызываемой ингибиторами EGFR, представляем современные данные о взаимосвязи полиморфизма гена и тяжести проявлений нежелательных дерматологических реакций.
