С 1982 года произошла революция в лечении акне, когда впервые для данного заболевания был одобрен изотретиноин.
Изотретиноин (13-цис-ретиноевая кислота) до сих пор остаётся наиболее эффективным средством для лечения акне. Системный ретиноид (производное витамина A) является единственным препаратом, действующим на все звенья патогенеза данного заболевания. Считается, что изотретиноин улучшает состояние кожи при акне путём понижения выработки кожного сала сальными железами, нормализации фолликулярной кератинизации и подавления роста актинобактерий Cutibacterium acnes (семейство Propionibacteriaceae) за счёт изменения фолликулярной среды и уменьшения воспаления.
Изотретиноин имеет доказанную высокую эффективность, отмечена также его хорошая переносимость, однако, как и любой лекарственный препарат, он имеет определённые побочные эффекты (дозозависимые и обратимые). Несмотря на то, что на протяжении почти сорока лет препарат широко используется в медицине, всё ещё существует неопределённость в отношении того, как лучше назначать данный препарат и какова длительность ремиссии.
Предлагаем вашему вниманию современную информацию о возможных побочных эффектах изотретиноина, обсуждаем пути их предотвращения и лечения.
