К вопросу о переносимости и безопасности фототерапии у больных псориазом

Аннотация
Обоснование. Фототерапия с использованием различных длин волн ультрафиолетового спектра успешно используется для лечения целого ряда дерматозов. Наряду с положительными эффектами фототерапии существуют как остро возникающие, так и отдалённые побочные явления, которые необходимо учитывать при ведении пациентов.
Цель исследования ― изучение переносимости и безопасности фототерапии (ПУВА и УФБ-311 нм) у пациентов с псориазом средней и тяжёлой степени течения.
Материал и методы. В период c 2005 по 2021 год под наблюдением находилось 920 больных псориазом, из них с вульгарным псориазом 756 (82%), каплевидным ― 36 (4%), экссудативным ― 73 (8%), инверсным ― 55 (6%). Псориатическим артритом страдали 249/920 (27%) человек. Пациенты со среднетяжёлой степенью псориаза (n=473) получали УФБ-терапию 311 нм, с тяжёлой (n=447) ― фотохимиотерапию ПУВА.
Результаты. За весь период наблюдения 385 (41,8%) пациентов отмечали ранние побочные эффекты фототерапии, в том числе один или несколько одновременно, которые были кратковременными и проходили по окончании курса процедур при соблюдении рекомендаций. Ранними побочными эффектами процедур ПУВА чаще всего были преходящая тошнота, рвота, головокружение, головная боль, транзиторное повышение печёночных трансаминаз; среди отдалённых ― лентиго (стойкая крапчатая пигментация), рост меланоцитарных новообразований в размерах, ПУВА-кератоз. Ранние побочные эффекты от действия ультрафиолетовых лучей представлены зудом, фотодерматитом в виде эритемы или пузырей, фотоаллергической реакцией в виде папулёзной сыпи, сухостью кожных покровов, фотоконъюнктивитом, простым герпесом и фотоонихолизисом; отдалённые ― появлением новых невусов, развитием ксероза кожи, а также фотодерматитом в виде эритемы, жжением кожи. У пациентов обеих групп выявлены признаки фотостарения. В нашем исследовании только у 1 (0,2%) пациента, получившего 5 курсов ПУВА (119 процедур), была диагностирована базалиома кожи лица, при этом прямая связь с проведением ПУВА была сомнительна.
Заключение. Ранние побочные эффекты при фототерапии кратковременны, проходят самостоятельно или купируются минимальной корригирующей терапией. Отдалённые проявления значительно менее выражены при УФБ-311 нм терапии. Установлен дозозависимый эффект фотостарения: чем больше получено курсов фототерапии, тем более выражены признаки фотостарения, особенно при ПУВА-терапии.
