Аннотация
В обзоре представлен современный взгляд на стресс как фактор риска развития артериальной гипертонии (АГ). Подробно описаны многообразные патогенетические механизмы повышения артериального давления на фоне стресса. Подчеркивается значимость активации симпатической нервной системы как ключевого звена в развитии стресс-индуцированной АГ и инициировании каскада патофизиологических реакций, реализующих свои неблагоприятные эффекты на уровне целого организма. Особое внимание уделено АГ на рабочем месте как варианту стресс-индуцированной АГ в силу ее широкой распространенности и ассоциированности с повышенным риском развития сердечно-сосудистых осложнений, в первую очередь инфаркта миокарда и мозгового инсульта. Представлены эпидемиологические данные и результаты мета-анализов последних лет, указывающих на высокую значимость рабочей нагрузки как фактора риска неблагоприятных сердечно-сосудистых событий. Изложены актуальные психологические подходы к коррекции воздействия стрессоров на человека. Рассмотрены возможности применения β-блокаторов у пациентов со стресс-индуцированной АГ, как препаратов, воздействующих на центральный патогенетический триггер данного заболевания. Представлены преимущества применения бисопролола как высокоселективного представителя класса β-блокаторов с учетом имеющейся доказательной базы его эффективности у пациентов с АГ на рабочем месте, а также с точки зрения метаболической нейтральности и органопротективных свойств.
