Ретроспективный анализ течения и исходов ювенильного артрита у взрослых пациентов

Аннотация
Цель исследования. Оценить течение и исходы, функциональный и социальный статус у длительно болеющих ювенильным артритом (ЮА) взрослых пациентов.
Материалы и методы. В исследование включили 213 больных ЮА в возрасте от 16 до 60 лет (средний возраст 23,9 ± 6,5 года) с длительностью болезни от 10 до 53 лет (в среднем 17,4 ± 6,8 года). Обследованы в Институте ревматологии 97 больных, остальные заполнили анкеты по социальному статусу и опросник НАQ.
Результаты. У 72,9% обследованных обнаружен полиартрит, у 17,7% - олигоартрит, у 9,4% изменения в суставах не выявлялись. Системные признаки отмечены у 3,1% пациентов. У 1/4 пациентов развился увеит. Функциональные ограничения суставов были минимальными или отсутствовали (функциональный класс - ФК I, II и НАQ = 0-1,0 балла) у 71-81,2% больных, нуждались в помощи окружающих лиц (ФК III, IV и НАQ = 2,1-3,0 балла) 18,8- 29%. ФК коррелировал с показателями клинико-лабораторной активности. Минимально или умеренно выраженная активность заболевания сохранялась у 87,8% пациентов. У 1/3 больных были выявлены эрозии, у 31,9% - анкилоз, у 27,8% - асептический некроз преимущественно головок бедренных костей. Состояние своего здоровья оценили как хорошее или очень хорошее 43,3%, как удовлетворительное 39,8%, как плохое 16,9% пациентов. Работали или учились 78,9%, окончили или продолжали обучение в вузе 48,2%, создали семью 31,7% пациентов, у 46 больных родились дети.
Заключение. У большинства взрослых пациентов, заболевших ЮА в детском возрасте, активность заболевания снижалась, отмечался благоприятный функциональный исход. Состояние здоровья позволило получить профессию, адаптироваться к повседневной жизни и социальной среде. Пациенты с более тяжелым течением и исходом болезни и формированием выраженных функциональных ограничений нуждались в лечении и продолжении врачебного наблюдения.
